
Step 1: Referral
Children may be referred by a medical professional, family member or any other therapist. Contact a Family Resource Coordinator (FRC) at 206-286-2322 to make a referral.
Step 2. Call to Family
A family resource coordinator calls a referred family to explain the program and make appointments.
Step 3: Intake/Assessment
The parents/primary caregivers discuss the strengths and needs of the child from the family’s perspective. The family resource coordinator and the therapist collect information to determine evaluation tools and services to meet the family’s needs.
Step 4: Evaluation
An evaluation using standardized assessment tools looks at the child’s development in the following areas:
Step 5: Individualized Family Service Plan Meeting
An IFSP meeting will occur when a child shows a delay in at least one area of development. At this meeting, outcomes are written based on family priorities determined during the intake/assessment.
Appointments are made with a primary service provider to support the family in achieving outcomes.
